Locate the "form builder" by searching in the "find rooms" search bar in the left hand navigation. Search for the form you wish to update, then click the pencil icon to edit and click"confirm".
Within the form, scroll the the end of the page and in the center panel click “add a new form field”, and then “from scratch”. The field type will be “Multi-field Autoselect” and input name should just be a one word name to identify the field (e.g. office.) Click "add field" and then "save."
Next, click on the 3 dots on that fields card. Add the input, post and search labels, then under “select data set” type the name of the data set (set of answers), and click on the "+":
Click on "confirm" to creating a new data set, then from the pop up window select “create new data”. Type in the “value” and “name” (they should be the same unless you want the answers in a specific order) and click on “add new data” after each answer option.
Continue this process until you have added all the options, then close the pop-up window by clicking on "X" in the upper right hand corner of the pop-up.
Make sure the “select data set” field for your new question displays the name of the data set you just created, and click “save”.
Upload Answers
If your answer set is too long to enter one by one, you can also upload a file. Once you have followed the directions listed above to start the dataset, instead of clicking on "create new data", you'll click on "upload csv file":
Your CSV file must have the the column headers as "name" in A1, and "value" in B1. Your answers will follow in lines 2, 3, etc.
Once your file is uploaded, close the pop-up window by clicking on "X" in the upper right hand corner of the pop-up. Make sure the “select data set” field for your new question displays the name of the data set you just created, and click “save”.
Order of Answers
The answer list will default alphabetically, however, if you want a specific order, you can use the "value" field to define the order (e.g. 1, 2, 3, etc.) and the the "name" field to display the actual answer (e.g. Chicago).
The value field is not seen by the end user. If you choose to sort by value, be sure to update the field order settings and check "order by value":
